For Referring Veterinary Hospitals

We sincerely thank you for your confidence in our hospital. If you are looking to refer a patient, please fill out the form below to provide us with additional information. Please inform the client that they need to call us to schedule an appointment. Both North Carolina and Oregon potential new clients can call us at (503) 574-4150. If you chose to provide records, we request records from the last 3 or 4 appointments (minimum) and all lab-work from the last year. If the patient has completed allergy testing and/or immunotherapy, please provide all accompanying records regardless of their age. If you will not be providing records at this time, we will call to request records after the client has scheduled their appointment.

Again, thank you for your referral. We look forward to working with you in the efforts of providing our mutual patient the best medical care possible.
